As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.Job SummaryThe Licensed Practical Nurse (LPN) will be responsible for delivering and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders for each individual patient. This role requires strong clinical assessment and documentation skills, as well as the ability to work effectively in a fast-paced environment.Key Responsibilities

• Deliver and document skilled nursing care in accordance with the developed care plan and physicians orders.
• Continuously observe and assess patient condition and care needs, reporting changes in condition to the supervisor and/or physician as appropriate.
• Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
• Participate in the development and update of the nursing care plan.
• Take appropriate nursing action based on assessment and achieve expected outcomes.Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.Preferred Qualifications
    • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
    • Home health experience.Physical Requirements
      • Must be able to speak, write, read, and understand English.
      • Must be able to travel.
      • Must be able to lift 50 pounds.
